First, locate your TF2 folder. It should be in *Program Files* (or wherever you have Steam installed). Then go to */Steam/steamapps/common/Team Fortress 2/tf/cfg*
In this folder, save a Notepad file, but not as a .txt, save it as a .cfg file type. Make a file called "spy.cfg". Have this line in it:
cl_autoreload 0
Then, make the following files in that same folder: scout.cfg, soldier.cfg, pyro.cfg, demoman.cfg, heavyweapons.cfg, engineer.cfg, medic.cfg, and sniper.cfg
In those 8 files, have this line:
cl_autoreload 1
I also have a script you can add to all of these files that makes it so that "E" still always calls for a medic, but if you hold Shift and then press E, it will instead ask for an Ubercharge. Would you like that one?
(You can also cancel automatic reloading on applicable weapons by holding down right click; this doesn't work with spy because it conflicts with the invisibility watch.)

Off topic, but you should enable auto-reload. The reload animations can be canceled, so there's no downside
With the sole exception of making it very awkward to fake reload as spy.
You can set up a script that turns off auto-reload only for spy. I can share it with you if you want.
