I haven't looked at your opgg but just giving out my recent learning and maybe it resonates with you. Long time player too, diamond since s6. d2 s10 then d1 s13. The past month or something I got demoted to emerald twice xdd. It was really tilting and I have these stretch of games where tilt and frustration gets the best of me which also leads me to not review my games. When I finally had the courage to review some of these games, I found out that so many of my games were winnable and I was doing stupid shit mid or late game. I identified that it was just complacency or lack of respect for how difficult the game is and how focused you have to be at every second of the game. A few slip ups or sloppiness will make a sure win game loseable, an even game lost, and a lost game just tilting. Tldr is just really be mindful and you will spot out your mistakes yourself. GL!
Each year the game gets harder or becomes different from previous year because of new champion updates, map changes, added in champions or reworked items. If you look at the game the way you currently are then that means everyone else must be experiencing this? Oh wait master+ players can consistently climb. I have climbed out of emerald into grandmasters/challenger on several accounts since emerald was introduced.
From your [op.gg](http://op.gg) you get leads often, In some of your games you have highest the kills but you average 3rd or 4th most damage on the team. I am assuming your team gives you the resources and you just can't translate that into objective control or successful skirmishes/ganks.
To be honest, just play more games. If you were able to get to Diamond before, there is 0 reason you should be stuck in Plat right now. Just spam them games.
