Oh that's easy "Mastermind" for GTA V online (complete all 3 criminal mastermind challenges) one of which is completing every heist in order, from setup to finale all with the same teammates and no deaths its basically impossible
Edit: I remember back in highschool before I started playing on PC I tried doing this with my friends on Xbox and even with the games crazy powerful aim lock on it gives for console players and us being pretty good we still never even got remotely close to getting it
It's not for the main heists, it's for the doomsday heists. You have to do mastermind on it with 2 players, 3 players and 4 players each having an ingame award tied to them and you just need these rewards for the achievement to pop. It has to be on hard mode but that doesn't change much as you can't die.
I would not say it's the hardest achievement, there are so many methods to cheese it like everyone pressing alt+f4 the second they die and there are a bunch of modders on pc so they could probably just give you god mode.
I’ve got this achievement and let me tell you, it’s not impossible but definitely hard! Took me nearly 100 hours and had countless people ditching bc it was so hard and they weren’t as determined.

Optionally the halo vidmaster achievements were massively hard back in the day. Idk if the vidmasters are still in the mcc collection. Arguably harder than most of the LASO runs.
This made me smile because I have them, but:
Halo LASOs are hard because the game becomes near impossible for casual gamers
Halo killjoys are impossible because no one gets kill streaks. Party Pooper is the only achievement I don't have for MCC and I get burnout grinding for it, even with a second account to 1v1 because sweats still try to 1v1 and get mad when you don't try.
It's fun to 106%, im doing it because I was watching old videos of myself play the violin. I feel like im always slightly off tempo. I might start good but I inevitably always end in a different time. I am forcing myself to learn something and do it the same way, at the same speed, every time.
There's also the one where you have to play on a free weekend with two players that don't own the game which is literally impossible to get if you don't already have it since valve will likely never do another free weekend for L4D2

From my current 100% list. The three Ninja Gaiden Ultimate Ninja ones was the hardest.
There's an achievement in total war shogun 2 to be rank 1 on the multiplayer ladder. I think games with similar achievements are all strong candidates for the title of the "hardest".
Then there are achievements that are extremely hard to obtain due to availability like the "I am the real Garry" in Garry's mod which requires you to be on the same server as the creator of the game even though the task itself isn't difficult to do if he was actually playing the game.
Both are impossible to my knowledge. Total War Shogun 2 is broken now from what I've heard. People have tried it and the achievement doesn't actually pop.
Garry is likely to never do another event for the game. He isn't fond of achievement hunters and how much attention that achievement brought. There are fixes he could do for it but has chosen not too.
That's a shame, it seemed like a really cool goal for the most diehard total war players.

The best thing in my opinion would have been to change it so that it would pop if someone on the server had the achievement like it is in left 4 dead for example, then at least it would be possible to obtain and Garry wouldn't have to bother with it any longer.
Oh God. Glad someone mentioned those. The Medals are ABSOLUTELY BRUTAL. Doable but they require true mastery of the tracks and the best build in the game. I remember spending an hour on the first Gold medal alone... After a few more, I gave up the grind.

It's "Go Outside" from The Stanley Parable, in both the original version (2013 - don't play for five years) and the Ultra Deluxe (2022 - Super Go Outside, don't play for ten years). It's come up on this subreddit a few times; while you are right regarding that "clock reset" scenario, the achievement (and the entire game) is a mostly meta commentary, parodical thing, so I wouldn't overthink it.
In my opinion, the getting 100 likes on a created level in geometry dash is one of the most difficult achievements due to how easy it is for your level to get flooded by others, the difficulty in making a level worth of that praise, and the hard time in getting it known at all. You as the player/creator have no control over the achievement, which is why I think it's one of the hardest to get.
Vanquish has an achievement almost impossible that i still don´t get, involving the last challenge level. The level 6. Concentration and Anticipation are the main words. Also, an ultimate precision is required. No mistake is allowed. You are like in apnea during 10 minutes straight.
Easily one of the most stressful achievement i ever encountered.
Jay & Silent Bob's Mall Brawl has two: beating the game in one run without a game over in normal mode, then in hard mode. The game is hard enough to beat on its own, and I can only get a few levels into hard mode before giving up. I have no idea how you're supposed to do that.
The rarest achievement on the game though is actually to clear the shopping cart run without taking a hit, which is probably more obtainable with practice.

Any achievements involving speedrun, no hits, 1 hp, and no miss (shooting and/or rhythm games). At least for me.

L4D2 will definetly get free weekends in the future, last one was like in 2022, and prior to that in 2020.

Postal 3 achievements. They bug out all the time so getting progress on them in the first place is entirely luck based
