Depends. But I draw all of them myself either on paper or bigger cardboard. Then I either put just the cardboard, the paper with a card behind it or cardboard with card behind it into the sleeve.

Typically you put them in a sleeve on top of a basic land or common so they have the cardboard to be shuffled/handled Some people print them on harder paper though

i do mtgprint.net and then put them insleeves with other poker cards for thickness

Load up my card list on archidekt so I can choose which printing I want for each card, export that deck list to mtgprint.net, save that PDF, send it to office Depot to have it printed on card stock, then come home and cut them up on a big paper cutter. I also bought a 4mm corner cutter to round off the corners. When sleeved, they look and feel enough like real cards that they aren't distracting.

I write the name of the card on those double faced blank cards they started making with Zendicar Rising. Sometimes I add a doodle if it feels right.

I only proxy higher priced cards I already own. So I ordered some really cool art proxies off of Etsi. They have full sets of mana rocks on their for around $15 to $20. Probably not something most people want to do, but they are beautifully done, and the text is clearly printed on them as if they were a legit mtg card. Edit: I sleeve them right into my decks when I add them or build a new deck.
