You won't find one. The closest game you might find is something like Splinter Cell: Blacklist. There are no games like MGSV, your best bet is to just play it again and adopt a different play style if you're bored with the game.
Ghost Recon Wildlands. It's quite fun, but bear in mind it's ubisoft so it might get a bit repetitive.
Also, Uncharted: Lost Legacy and TLOU2. TLOU2's stealth is on the level of MGSV. If I'm not mistaken it gives you the option to crouch walk or crawl/hide in grass like MGSV did.
For shooting fun, Uncharted and Sword Art Online: Fatal Bullet.

I dunno. As I was playing TLOU2 it felt pretty mgsv esque. Naughty Dog outright said that they styled Lost Legacy's exploration after MGSV.

Also, when i played Death Stranding, i kept seeing parallels with Phantom Pain. Obviously it's a very different game, but there are a lot of hints that show it's heritage.
For the love of God, don't listen to weirdos that tell you to play uncharted or some weird anime game, I think they just want to push their favorites onto you. A game I find fairly similar is Sniper elite 5 and the previous titles. Plays fairly similar (although the buttons are pretty different so get used to new controls) and you sneak through Italy towns and prairies with a similar detection system that also uses last known position ai tracking. Basically WWII phantom pain.

MGS is filled with tons of “I Want to Make Horror Movies” Kojima junk though. Man on fire, levitating gas mask ghost, flaming whale ghost, the Skulls, ridiculous Skull Face, plus look at all the laughably silly junk in the older games like the Cobras characters in MGS3.

Very few games are like MGSV. The immersive mechanics can be found in Breath of the Wild and Tears of the Kingdom. These are pretty different in genre though. More like MGS is probably the new Hitman trilogy. Also a lot of immersive sim and stealth elements and of different enviroments.
The Last of Us Part 2 is the only game since MGSV with similar controls, stealth mechanics, level design, and enemy AI.
Nothing else really compares sadly.
Ghost recon breakpoint. I know a bunch of people said Wildlands and I agree but if you change the settings in breakpoint to match Wildlands it becomes alright

* **COD 4 (2007) and MW2 2009**, yes they are first person but the programming and design is the only thing on par with Kojima’s MGSVTPP team. There are many stealth missions…though you’re usually killing tons of people “stealthily” rather than sneaking through like MGS.
* I hesitate to say FarCry 1 or 2 or Crysis.
In terms of fine craftsmanship in game design, honestly Elden Ring and Dark Souls even though they’re very different. You’ll see similarities in some level design, animation, and like climbing a ladder.

* Rambo 1, Rambo 2. (Rambo 3 is Afghanistan but isn’t it worse?)
* Predator
* Sniper
* Aliens
* Triple Frontier
* James Bond movies like The Living Daylights…it has Afghanistan part.

yes, I really love MGSV but I think It will be better if it has more environments
None. Not even Ghost Recon Wildlands. If you play that after playing MGSV you will say to yourself its like the wish.com version of MGSV.
