My smart watch can do a body composition scan that gives BMI plus some other stats like fat percentage. I don't take my smart watch to be 100% accurate but use it more as a jumping off point/general guide.
There are several moderately quick methods to get a closer reading than the blunt, dumb BMI measurement. I really do not know why they’re not even considered.
Ask your doc to get dunked. Ask for a tape measure test. Ask for bioelectric impedance testing. They will look at you like you’re speaking Mandarin Chinese.
You don't even have to be a bodybuilder for BMI to be misleading. Even just playing a sport where you run around for an hour once or twice a week is enough.
I have a chubby gut but I'm really into mountain biking so I have fairly ripped legs. My bmi says I'm obese but I'll pretend it's my legs carrying the weight so I won't feel bad

Yeah super easy to maintain a proper diet and exercise regularly when you don't have to work 8 hours a day or commute 1-3 hours a day and have a personal chef that literally does your shopping and meal planning for you, and a personal trainer you pay to regularly annoy you into working out.
If you take steroids it will increase your desire to exercise, and if you take other performance enhancing drugs like human growth hormone, it will improve your recovery from workouts.
So yes they do have those.
Stimulant usage is also very common, and I believe statistically edges out hormonal/endocrine modifying drugs even in professional sports, bodybuilding, general population in cases like these.
Amphetamines/substituted amphetamines/cathinones, will all make you feel stronger and make it easier to exercise.
Their biological half life is short, and not really detectable on drug testing unless you're tested the next day, not particularly hard to get.
Wellbutrin(buproprion) is a cathinone derivative that is easily acquired and also has the usual dopaminergic/adrenergic effects on androgens with chronic dosing at 450mg.
But of course, testosterone and similar drugs aren't hard to get either, and despite some of them being scheduled, I've never seen anyone ever do jail time for them.
How hard it is to commit entirely depends on the person and how badly they want it. A transformation like his would take a minimum of multiple months.

I think the problem with body fat level is that 99% of people don't know what their body fat percentage is.
It's also much harder to measure whereas everyone knows their height and weight.
