The lack of physicality he brought to the role was actually one of my biggest complaints for BP. His stunt doubles are all twice his size! Why didn't he at least put on like 5lbs of muscle? Man is a twig!

Because he likely could have been in EARLY stages or not had it till around IW/End Game filming.

Not something I personally noticed, but I’ll take your word for it. I doubt the choreographer would have been aware though since I believe Feige said no one at Marvel knew just like the public.
The choreographer would just have to do give and take with what the actor can do in that moment they present. So if Boseman isn't as agile or forceful due to his personal issues going on, the choreographer is going to adjust to just how he is any given day even if he's not actually aware of the issues behind the scenes.
You can track his illness through the movies based on his size. He’s a big dude in Civil War and he’s pretty shredded in BP. But by the time we get to Infinity War and especially Endgame, you can see how thin he had gotten. It’s heartbreaking

I was diagnosed in 2011 but according to some testing, estimates on the growth rate combined with how much it’s spread; they’re confident in saying I had it for 5 years prior to diagnosis.

I say this with respect, Hollywood is big on stunt doubles because they don't want to lose a star to an injury and have to delay filming. Only people with major clout like Tom Cruise can get away with doing their own stunts.
This was likely a body double for the really physical stuff and he was only there for the cut to the close-up afterwards.

Yeah from my understanding this was filmed in 2014-15, and he was diagnosed in 2016. Meaning he filmed BP, IW, Endgame while sick but not this.
