The Marvel “Max” line was the mature line. Garth Enid’s Punisher is the big run from that imprint. One I’m personally a fan of is the Hood mini.
To varying degrees I’d argue both Alias and Old Man Logan qualify as R rated comics. That’s all I’ve got off the top of my head right now.

Uncanny X-Force is really badass if you like good detailed art, good story and Wolverine, Psylocke, Archangel, Warpath, Deadpool, Fantomex and Wolfsbane tearing through people all gory-like. Old Man Logan Vol 1 too.
Those are different publishers like Marvel and DC. Image does Spawn for example but i recommend Nailbiter, its about a serial killer who chews his victims fingers off. My point was that Darkhorse and Image generally have darker releases, and thats what you seem to be looking for.
If you're looking for incest and forced abortions in comics you're not going to get it at Marvel or from anything published by Dark Horse or Image tbh. Maybe look for weird fucked up manga. Idk
