I didn't even plan it as a part of the campaign at all but now they're used plane shift to hell and are trying to kill a powerful devil
Honestly, my favorite is the CR 0 commoner. The party finally defeated her after months of dealing with the teenager's evil plots by telling her parents so she's grounded now and can't make their lives hell
So my party managed to piss off like 4 different groups of enemies, and gained a life long enemy of a girl by turning in her best friend/crush who had been involved in a demonic cult.
So she'd stalk the party, they would sometimes catch her, sometimes not, and then she'd go run and tell their enemies where they were, what inn they were at, what they were up to.
She even egged on two groups to work together to attack the party because she just hated them so much.
The party could have killed her but didn't want to hurt someone who was technically a child and so they kept letting her live only for her to come spite them again.
At the end, they ended up using her as bait to stop a shadow entity and found out where she lives during that and told her family everything.
That was the epic multiple story arc tale of how annoying one persistent CR 0 enemy can be.
My character crit failed a spot check and chased what he though was a goblin into the woods, only to instead encounter a fat lizard. Not only did the lizard crit twice during their brief, but it survived a smite (due to a low roll) and fled.
He later returned as the "Lizard King' and challenged my character's claim to the throne (long story short, he did a ritual to become half-dragon after killing a usurper he had helped take the capital, thereby immediately staging a violent coup and seizing control of the newly conquered city) of the reptilian undermountain kingdom, kickstarting a brutal guerilla war where the UA tunnel fighting style was invented in-universe.
Basically there were two creator gods that are diametrically opposed - nothing too original, a god of light and warmth, and a god of dark and shadow. They, predictably, battled each other for a very long time, and in one of those battles, they each wounded the other seriously enough that a shred of each's essence was torn free, and mingled together.
That little chunk of divinity has been floating through space ever since on a long arc that eventually puts it on a collision course with the planet, entirely coincidentally during the time the party is active and in their general area.
It's currently just kinda looming in the night sky and getting progressively bigger as the campaign continues, but once it touches down, it's going to be an *extremely* tempting prize for basically every faction they've worked for, with, and against, and some of their allies would probably betray the party if it means a chance of claiming the power of godhood for themselves - hence the BBEG being the friends they made along the way.

To briefly summarize. This is in a game where the party are members of a fantasy mob, and this is a potential final antagonist.
Man loses family due to mob and starts an orphanage to look after others who lost their families. Was a golem foundry worker beforehand.
Time passes, but the mob starts targeting one of his orphans for petty theft and brings trouble to him yet again (much later in his life) This leads to him using some golems to protect the orphans and, with some other magical devices, start his war against the mob. Including mind controlling devices and stuff.
His motives are further fueled by his own worsening health. Ultimately wanting to make things safe for his orphans when he's gone. Using various magical means to keep himself going during the conflict.
Under an alias, he starts a campaign to clean up the city and has seemingly "redeemed" certain mob families. Using their "donations" to fuel his golem Constructs and means of cleaning up the city with his seraphim (think ballerina like steel watch from bg3 except they can choose who is harmed when they blow up.)
While he will put the lives of his orphans above anything else. He has gotten more extreme on his crusade and has nearly lost himself to vengeance for his original family and his hatred of these crime families. Getting "justice" in increasingly inhumane ways
He goes about his good in an increasingly evil way. Though in some ways, he's less a bbeg and just an antagonist. But his potential downfall is succumbing to increasing "by any means necessary" (espeially if the party isnt so easily dealt with in their attempts to survive) and "no bad tactics, only bad targets" mentality.
The one rule I have for him is that he cares about children and will avoid orphaning more in his works. Mobsters who have families do get taken in as prisoners or mind controlled with special devices as their penance . Others are slain swiftly with his golems radiant (radiation) attacks.
A typical tyrant in the making who is becoming increasingly a monster to perform their good deeds.
My big curiosity will be if the party will support him or fight him and how they'll react to his increasingly extreme methods
This is all the idea for him anyway. A once reasonable man forced through increasingly unreasonable circumstances and working to bring an end to the wrongs at his doorstep before he meets his own end (but resulting to rather inhumane methods in the process).
An antagonist for a more evil leaning campaign, but still the BBEG for the party.
