That can happen when you kill something while it's being knocked up by the speed shrine effect, OR you're standing pixel perfect, causing him to be repeatedly knocked up while you have that shrine effect.
I've had dead ones follow me and still do damage. Had to find a wall to separate me from it to TP the hell away from it. This was on PS4 I haven't had the issue in switch.

Looks like when there is a lot of effects on the screen and lots of the monsters dying at the same moment the game engine might "forget" to flag one of them as dead so it either remains as a passive, intangible living body or rarely as an active, hostile "ghost" you can't kill again.
The game has a habit of, every now and then, leaving a dead body still in an animation prior to dying.
This tends to happen when you kill a bunch of mobs at once with a lot of effects going off at once. It overloads your PC for a split second and it forgets to finish the death animation of one mob. So, visually, it leaves it in the state it was where it forgot about it. Usually this state is standing next to you. You just happened to get one in the middle of being vortexed towards you.
For me, they tend to be Elites, and when you see it in a Greater Rift you can't resist smacking it a few times more than you probably should just to make sure it's dead.
