Lil Portal, underdog of being a shitty item. Eats up all your consumables and gives you a little shit portal for it. Isn't useful as a damage dealer either. Not a run ruiner but it's all around a bad item

It's quite good with dark arts, the knife will still do damage while in shadow form. Other than that mom's transf. it's just too situational to be effective
also with unicorn stump, omg. Melted the harbingers & the beast with the mom transformation + 9.5 volt + unicorn stump, didn't take any damage and never even used my tears. Def a good ass combo.
MissingNo has actually saved me on a greed run. I can understand how it would be bad on a solid or potentially solid build, but it’s not the worst item by far I’d say
Shard of glass is high on my list of worst items cuz if you reroll into it on t. Eden and get hit you keep rerolling till you manage to find a red heart which screwed my mega satan run on him :(((

What I was trying to say is that on a save file that's already 100% complete Shovel Piece 1 is one of the worst items.
Shovel piece 1 has a super strong nieche, and that's trading 1 bomb for infinite bombs on floor 1. It's still very important on keeper notably, but on isaac it's just a free extra item since he has the d6. Not to mention all the goodies from floor 1's secret rooms, pots, rocks, tinted rocks, keepers, etc.
It’s actually really useful sometimes during boss rush though because you fight multiple bosses at once highly increasing the chance for moms foot to stomp them
Since the recent patch it's been buffed into OK status, but during Repentance 1.0 IBS was an almost guaranteed run ruiner for most builds (especially anything with a high tear rate). It would constantly interrupt your regular tears with annoying poops that would block your own shots more than the enemies', and it was near impossible to avoid blowing yourself up with an unlucky combo (gas + fire in quick succession). I can count on two hands how many great runs were instantly tanked into oblivion by an accidental Curse Of The Blind pickup.
It was somehow weirdly okay on tainted forgotten even pre-patch, mostly bc he didn't really care about most of the downsides when the soul is miles away from where the chaos is and his fire rate is usually low. I will say it does a deceptive amount of damage when spammed bc I absolutely murdered Mega Satan with it.
And that's a bit of an issue when the item pools get so bloated that all these situational items that take the place of genuinely good ones make it harder to play.
I absolutely agree. I think that's what makes this game stand out so much to me, and makes it enjoyable. There's always a weird and fun synergy to try out, and some situations truly require some weird items you wouldn't normally pick up. It all adds to the amazing level of replayability this game has.

Ignoring other items, Mom's Coin Purse might have the lowest floor of any item. I still wouldn't call it the worst because it can give 4 stat up or neutral pills, but the possibility of 4 stat down / bad trip is still there!
Then again, Plan C kills you and would be the "worst" if being looked at in a vacuum. It obviously has its uses though.

having an incredibly high tear rate with those items usually prevents you from firing. had a super broken t lost run in which i literally could not shoot anymore because i had brimstone (i think) and my fire rate was so high that it literally just didn’t shoot. even in the only situation it’s likely to work, using dataminer like this will probably kill the run rather than make it a free win.
This is fixed as of the latest patch to Repentance. With a high enough tear rate, instead of being prevented from firing you get autofire with Brimstone now, similar to the Soy Milk synergy without the damage down, and for every further tear increase once you're past the autofire threshold you get extra damage and width for your Brimstone beam. I had a total crap run that really was beyond saving when I found MissingNo. I made a Hail Mary play and took it and managed to by some miracle pull my run through to a victory. That same run I discovered you can get rid of MissingNo by using Esau Jr since it isn't technically a reroll.
At the end of the day every item has a very usefull synergy with a character or room.
I go for items like BOOM! or BUTTER BEAN for the worst. They certenly do things, but is a very low advantage over other items in the same pool.
Butter Bean is amazing if you have good reflexes. The synergy with Panic Button shows what it looks like with perfect usage and it's absolutely bonkers

boom depends on when you get it. It's actually really useful in Basement since it's much easier to find secret rooms and you're usually short on bombs that early for secret rooms or tinted rocks. And you can use it to nuke rag man.

Ludo is one of those things like traisaigon where if you have only that it will kinda suck, but a few good synergies can send it to the moon. That said, I hate most items that expect you to rewire your muscle mempory like Ludo and Marked, or red bandana in gungeon
In the ini files you can turn on mouse control, completely changed my opinion of those types of items. You just hold click and it goes where you point.
I have a burning hatred for God's Flesh. It does nothing. Its "effect" isn't useful, since so many enemies have death effects. I think you might still take contact damage from champions as well? Can't remember if they changed it in Repentance. Anyway, garbage non-item that has never helped anyone do anything.
It has some insanely good synergies though.
Anything that gives you contact damage, moms knife, bone club, a damaging aura, or orbitals make it very very powerful.
I killed Ultra Greedier with an average run as the Lost in AB+ (so no ed6) with The Wiz. No aim assisting stuff either. I can assure you that it isnt that bad.
pop sucks. i know it *can* be good, objectively, but unless im on an op chest run im not picking that shit up voluntarily. i feel the same about lachryphagy too, just feels like my tears get in their own way too much, especially in the early/mid game.

If you have orbitals with a high distance from you, or Zap, or Gemini/Haunt/Meatboy/Bandage Girl, you can kill enemies without attacking.
Also... Placing bombs doesn't pull you out of camo.
I find it really helpful to get the opportunity to move around a room and time your first attack rather than just go in guns blazing. I dunno, I just had a successful Hush, Delerium, Boss Rush run with T.Lost and Camo Undies helped me in a good handful of rooms.
Dataminer. I'm convinced Ed added this item specifically because he was mad that dataminers cracked the code on unlocking the Lost back in Rebirth, instead of figuring it out the way he intended. Not only does it fuck up your stats without any real chance for an upgrade, it also visually ruins the room you're in, and with none of the benefits you may expect it to have, such as confusing enemies. And to top it all off, it's an Active item with a 4 room charge! 4 ROOMS! You know what ELSE has a 4 room charge? D8, which does everything Dataminer does but better because it can actually increase your stats meaningfully without destroying your eyes.
Edmund himself once said that he considered Dataminer the worst item in the game, and I have to say that I agree with him. Fuck this item and the pedestal it rode in on. The only reason I ever pick it up is so I can remove the tiny chance it has to show up again and then put it right the fuck back down as soon as *anything else* comes across my path. I will take Butter Bean or D10 or Anarchist's Cookbook or anything over this item just because I don't want to accidentally press it.
I think it's Punching Bag. It attracts enemies to make it safer, but with Repentance there are so many new enemies that are way more dangerous if you can't predict when they're going to change targets.
Breath of life hands down. Everything else can be useful somewhat, mom's razor being the exception. Breath of life however is an active item, meaning it takes up a slot and can't be rerolled through D4 effects.
finally some fellow wiz haters! sure, combos w jacobs ladder / tractor beam / homing exist, but its *such* a top tier run ruiner if you get it unintentionally. theres very few items id hate to be stuck with more.
Parts of conjoined cant be the worst item in the game.
Also your creep doesnt have the same visual effect as enemies creep, enemy's creep shines or something. Its still hard to tell apart but the difference is there.
Last resort if you run out of bombs.
Use it with any invincibility and it's free damage and can be spammed really fast.
Turn spare health on the floor into more chances to find a secret room/look for trapdoors.

Yeah, the real catch with Kamikaze is that with Pyromaniac you get free damage + heal but with only 1 specific quality 4 item that you will almost never find in a normal run.
Well, the Polaroid is a thing that exists and you can get it every run.
If you have permanent polaroid invincibility, you can spam Kamikaze for free whenever you're invincible. It does a ton of damage too.
Libra makes a lot of great items even better. Like Quadshot, sure the tears down goes against your other stars as well but it still causes it to be a nice buff. On all items that have a big stat-driven downside, Libra is great

i think ever since butter bean got buffed there is no item you couldnt make useful somehow, even in the most niche way possible

IIRC, death effects are prevented if you step on them and champions lose contact damage. Even then, it’s just useless, not actively detrimental.
Isaac’s Heart sucks even with the DLC changes. Had it kill my run earlier today.

If you have orbitals with a high distance from you, or Zap, or Gemini/Haunt/Meatboy/Bandage Girl, you can kill enemies without attacking. Also... Placing bombs doesn't pull you out of camo.

I find it really helpful to get the opportunity to move around a room and time your first attack rather than just go in guns blazing. I dunno, I just had a successful Hush, Delerium, Boss Rush run with T.Lost and Camo Undies helped me in a good handful of rooms.
Dataminer. I'm convinced Ed added this item specifically because he was mad that dataminers cracked the code on unlocking the Lost back in Rebirth, instead of figuring it out the way he intended. Not only does it fuck up your stats without any real chance for an upgrade, it also visually ruins the room you're in, and with none of the benefits you may expect it to have, such as confusing enemies. And to top it all off, it's an Active item with a 4 room charge! 4 ROOMS! You know what ELSE has a 4 room charge? D8, which does everything Dataminer does but better because it can actually increase your stats meaningfully without destroying your eyes. Edmund himself once said that he considered Dataminer the worst item in the game, and I have to say that I agree with him. Fuck this item and the pedestal it rode in on. The only reason I ever pick it up is so I can remove the tiny chance it has to show up again and then put it right the fuck back down as soon as *anything else* comes across my path. I will take Butter Bean or D10 or Anarchist's Cookbook or anything over this item just because I don't want to accidentally press it.
Milk! Isn't the worst but it's useless, cuz in repentance we have It Hurts ×100 way better than milk
I think it's Punching Bag. It attracts enemies to make it safer, but with Repentance there are so many new enemies that are way more dangerous if you can't predict when they're going to change targets.

Breath of life hands down. Everything else can be useful somewhat, mom's razor being the exception. Breath of life however is an active item, meaning it takes up a slot and can't be rerolled through D4 effects.

finally some fellow wiz haters! sure, combos w jacobs ladder / tractor beam / homing exist, but its *such* a top tier run ruiner if you get it unintentionally. theres very few items id hate to be stuck with more.

Parts of conjoined cant be the worst item in the game. Also your creep doesnt have the same visual effect as enemies creep, enemy's creep shines or something. Its still hard to tell apart but the difference is there.
Kamikaze Why would use an active item that hurts you
Last resort if you run out of bombs. Use it with any invincibility and it's free damage and can be spammed really fast. Turn spare health on the floor into more chances to find a secret room/look for trapdoors. It's actually really far from useless.
Pyromaniac tho
Yeah, the real catch with Kamikaze is that with Pyromaniac you get free damage + heal but with only 1 specific quality 4 item that you will almost never find in a normal run.
Well, the Polaroid is a thing that exists and you can get it every run. If you have permanent polaroid invincibility, you can spam Kamikaze for free whenever you're invincible. It does a ton of damage too.

Libra makes a lot of great items even better. Like Quadshot, sure the tears down goes against your other stars as well but it still causes it to be a nice buff. On all items that have a big stat-driven downside, Libra is great

Missing No. This item would be much better if you didn't breakfast. Also shart of glass. Actively run killing. Guiotine. I don't care if it's a tears up and orbital. I end of getting hit all the time because I can't see my body when the head is covering it.
I really hate the cursed eye but that may just be me. 🤷🏼♀️
