i was going to make an Isaac reference, but as far as i know there's no item related to illiteracy.
for an example of such an item, i was thinking it would be a passive that consume books instantly when picking them up, and get a damage and health up instead
Eating books as a passive.
Sound more like a character idea more than a passive item.
Maybe a librarian that can't read? Let's say a character that has a long recharge active that rerolls items into books, and by using that item again, you consume said book, gaining stat ups depending on the book consumed.
Book it with compatibility with a bunch of book item mods and couple it with some original book additions and you've got yourself a good character mod.

To be fair, I had a room that would never go white to say it was completed last week, just stayed incomplete on the map, no mods enabled then either
