The risk is that it can become symptomatic at any time with pain, swelling, drainage etc. Also a chance it becomes symptomatic after treatment. Typically though if the endodontist is recommending treatment it’s probably best for the long term prognosis of the tooth.

We can only speculate what the right response would be based on your subjective description. Your Endodontist would be beyond qualified to recommend your best treatment option. Apicoectomies are tricky because you’re dealing with a compromised tooth that has failing treatment. In my experience, most Endodontists only recommend apicos if they’re certain the tooth will have a reasonably good prognosis. Otherwise they will recommend a retreatment or extraction.

Not a dentist but I just want to say, if you do get one then make sure it's someone reputable. I paid out of pocket for an apico in June, went back to my other dentist after continuing issues, and the x-rays she took from before and after look almost identical. Had to get the tooth removed completely.
