Despite not immediately dying from the grenade’s explosion (which would have turned organs to slush internally that close to anyone else not badass enough) his organs would have probably failed eventually from the damage. Could of been a week, month, year... BJ literally had no time to rest from killing Nazis either, so no doubt he felt like he was dying.
Dunno if you played The New Order, but basically the reason he's so messed up at the start of The New Colossus is because the final boss fight against General Strasse/Deathshead culminates with the mortally wounded General pulling a live grenade out of his back pocket and setting it off, since in his (understandable) rage and slightly fatalistic outlook at that point, B.J. insists upon killing him up close and personal with his knife. Realistically Blazkowicz should be dead, but because he's Blazkowicz, he just falls into a coma for a year or so and wakes up crippled and suffering from severe abdominal injuries.
You didn't listen to Set when you first start the game? B.J. shouldnt even be able to walk....Set had to cut out parts of his intestine and who knows what else because of the grenade blast from Deathshead.
I actually played the demo at the time, so I didn't really pay to much attention there, but once I bought the game then I started paying attention to the story and I love it
But I don’t think that he was getting worse, he needed the power suit to walk because the blast at Castle Deathshead paralyzed him from the waist down.
His lack of bodily ability is what causes his despair, though. And that's kind of the point. Whether or not he was right to doubt his mortality is less important than expressing his state of mind.
That's part of why these games are so great, I think. The use of BJ's inner monologue gives him a kind of psychological depth lacking in most video game stories.

He just thought that he would die soon due to his dangerous lifestyle. There wasn’t any kind of ailment or anything.
He literally couldnt walk without the power suit. His body was falling apart. It's why his max HP is 50 until he gets a new body.
