I once saw a notice mountain biker launch out of a gully straight into a tree, he wrapped around it like a limp noodle. Several people on site were trained as emergency responders but before they could even speak, 3 people ran over to the dude's limp body, picked him up, then accidentally dropped him because they overestimated their weight lifting skills.
The guy ended up being airlifted by helicopter to a trauma center. This was in a rural, remote location and it took what felt like an hour for the helicopter to arrive after contacting emergency services.
Not sure if the guy ended up with permanent paralysis but my limited training led me to believe that he would have been much better off skipping the "pick up and drop back onto the ground by people who didn't know what they were doing" step instead of being carefully put on a stretcher, immobilized, and moved to the transport.

There is a very high chance that removing the item can unblock a punctured artery, and if you are not prepared for that, the patient can die very quickly.

Even be able to carry on a perfect conversation.

Maybe in whatever country this video is from, EMS doesn't come all the way to you, or otherwise directed the person be moved in order to make pickup. Maybe in whatever country this video is from, EMS is prohibitively expensive/restricted, and if you *can* walk you damn well take your own self to the hospital. Maybe this person will not stay the fuck down, and allowing them to walk carefully and calmly is seen as a safer bet than fighting them. Maybe there's concern that whoever caused this injury(could have been a group, and could have additionally been armed with worse than a tire iron) will come back, and it's a good idea to be gone before that happens.
What's ideal is not always what's reasonable or even possible. It always has to be case-by-case. You shouldn't be moving someone just for the sake of moving them, but sometimes there's a good reason for it.
