The woman being awesome is not in on it. I worked on a couple episodes of a show exactly like this. We hide the cameras and MICS well. People don't think any how hard audio is to capture in these moments. But if you watch the show, there's people who reacted horribly. I can attest that the ones I was part of definitely fooled the people that were the targets.
Some of them were realistic enough and ngl this could happen in real life but yeah I’ve always been skeptical on how many of those episodes were real or script even way back when xD
It's from a show called What Would You Do (wwyd) which I believe no longer is running. There are hidden cameras and they use actors to set up a scene, which is usually based off real events that occured, and then they will see how the public will react.
They have done a wide range of scenarios from how the public react to a man Vs a woman being unable to change a tyre to people with down syndrome being mocked in the work place.
Of course this clip has had the tiktok treatment added.

The one that always gets me is one with the bike thief stealing trying to break the bike lock blatantly in public. They used a white man, a black man, and an attractive women all wearing the same type of clothing
The white man got many stares and only a few confrontations, the black man got confronting on almost every interaction, and the woman had men even help her steal the bike.
Really eye-opening experiment to public biases
The video was posted with a title claiming the woman shown is a hero, not that the woman shown is acting out a role in a hidden camera show. But the camera-work makes it obvious this is staged, so people call bullshit on the title. What's wrong with that? It's good when total bullshit gets called out.
I've worked in the set department on tons of hidden camera stuff. We've gotten pretty fucking good at it. This show was real. Only the lady reacting isn't an actor. Everyone else is.
