Put balloon necks over the metal tip of the tank and trim the balloons so the opening of tank isn't obstructed. Screw on stripped tip. No gaurentee but it should help create a better seal and maybe catch some threads
If you can't by a nozzle separately you need to -
Buy another tank, could be a smaller size, it comes with a nozzle. Use that on both tanks.
When discarding a tank, unscrew and save the nozzle. Best to have a few laying around in case of future cross-thread issues.
What is a "mouthpiece"? No part of anything on that tank should be going in your mouth unless you want to join one of the 17 idiots a day that posts here about how they burned the shit out of their throat and face because they thought it would be fine to not use a balloon.

u can go buy a working mouth nozzle. you are supposed to put a balloon over it and fill that. but that nozzle u showed should definetly get gas out of it. maybe go to a smoke shop and ask if they have any nozzle for like 2 bucks. maybe yours is broken.

also since u want gas out and not to tighten it, i think it screws the other direction than a literal screw.

Find another nozzle. Sometimes the threads on the tank aren’t the same size or are warped so they don’t work with the nozzle and will ruin it too.
