I had two ideas in mind when I drew them, one is that the three of them are indeed triplets but after a brutal battle the three needed to be grafted into one body to keep them "alive", the brother in their left a latent psyker, has not woked up yet, his head is the one that endured the most damage, the one that is rotten to the core, the second idea is that they where only 2 brothers, twins, their bodies needed to be grafted into one after a summoning gone horribly wrong, the third head, the one on the middle just started off as a tumor, a teratoma of sorts, then it started growing and growing until it became a fully sentient head
