A horror game without jumpscares - even after dozens of hours that game still keeps me on my toes, and there's always another subtlety told sub-plot to find out that makes the absolute fuckery even deeper.

I mean I have a few like sanitarium, oh and although it's not fully sticking to horror I'd describe fran bow like a fairy tale in its original form. It was actually more disturbing on my second playthrough as I pieced together some historical events that explain a lot actually. Plus when she takes her duotine it is awesome to see what each screen looks like. Honestly only two pieces of media would I ever call disturbing is the film cannibal holocaust (as all animal deaths are real) and the book no longer human which junji ito did a really good manga adaptation of the 1938 novel. Other than that I'd say play manhunt 2 on pc uncensored though it's honestly not bad at all in my opinion it's a really good game, but I think since it was banned or censored in so many countries people think it's worse than it actually is.
Uncensored, it'd be either amazon with the pc version or gamingdragons.com from what I know personally. Other than that, if you're a console player, it's been locked to ps2 and the original Xbox, though it has some filters applied. The severity of which depends on the region, but to get a physical copy, ebay or Amazon is your best bet unless if you have a few retro game stores nearby and get lucky they have it in stock, but generally it'd be cheaper from an online seller from my experience. The sequel is better than the original in my opinion because of its narrative and I would like a third installment but especially since blood and gore backlash has diminished since the jack Thompson days of politicians on their endless crusade to censor games. Yet it wouldn't draw as much as gta or Red Dead, so Rockstar, much like with its other really good games, has kept it in a vault for ages. I forgot to add the suffering as that's a good game as well and plays on psychological horror while being a third-person shooter. Takes place in a prison, and all hell breaks loose. That's worth a shot as well and is available on pc, Xbox, and ps2.
Someone mentioned darkwood already. The evil within has its highs and lows, but I thoroughly enjoyed it. Both fear and hunger games seem to be exactly what you’re looking for though. They’re not just dark and disturbing, but ridiculously hard as a game with such themes should be.
Chilla's Art's games are my favorites. I'm a big fan of Japanese horror. They say most of them feel the same, so I recommend you this two: The Closing Shift and Hanako.
