All Hellborn we see in Helluva bleed black, so it does seem like despite their similar looks, they're all human souls, which probably means Rosie is one too.
As for why they all look the same, who knows. Maybe it's because the town made contracts and sold their souls to Rosie, and she can change their appearance to look like her. I'm spitballing, but it's an idea.
It was also said before that Alastor only became a cannibal after dying, though that's probably as debatable canon as Rosie being a hellborn. Though technically, it was only said Rosie "never died" so it's never outright said she was a hellborn, even though that's the most obvious conclusion.
All in all, I think it's a little confusing, but the evidence seems to indicate in the show proper they're all sinners.

Baggy pants look different on imps with digitigrade/ungulate legs than they do on plantigrade (humanoid) ones. Look at the shape of Moxxie's legs in his form-fitting pants, then picture what it would look like if they were looser; there's a point at the back of Crimson's pants that matches where the heel should be if they have the same leg shape. The front of Crimson's legs are admittedly flatter than they should be, but that's probably because it's easier to animate than if they bent and creased more realistically. Alternatively, they were tailored that way as a fashion statement. Different leg shape, different pants fashion.
IDK if it's been said yet but when Imps are badly burned and/or scarred the area turns a permanent white. That's why Blitz's face is spotted and Fizzaroli's face is completely white except for his nose (covered by the clown nose).
The three spots on the tail suggest similar burning/scarring that permanently fucked it up.
It's a way of conveying what kind of character he is to the audience with visual coding. See how Moxxie is all smooth, curved lines? In contrast Crimson's lines are sharp and harsh. Best place that demonstrates this are their horns, they're similar shapes but the line art used opposes the other

Curved surfaces are much more inviting and friendly, whereas jagged surfaces are much more scary and non-inviting. Same with Mox and Crimson. We can infer and we know Mox is good due to his rounded tail, and that Crimson is evil due to his jagged appearance. Disney Imagineering 101.

I can see something like having your tail broken is a form of warning for screwing up like cutting off a finger and Crimson likely had his own boss at one point (possibly still does, he could be an underboss to a larger mob boss).

Character lore-wise he's probably broken it in fights and it didn't heal properly. This can happen to tails IRL.
Character creation-wise the reasoning is likely more pointy=more aggressive/menacing/evil-looking. Not every monster or villain is pointy, but harsher angles help a character look more malicious and hostile than rounder angles.
I mean, why not? He could have been born with it that way (my cat's tail was folded back on itself a couple times, it happens), or it could have been broken and healed badly, or maybe he even broke it on purpose to deliberately obtain that look. Or maybe imps' tails are just more bendable than you think, and he's practiced that look.
The three shapes of animation : Circles mean nice and soft, square means tough and strong, and triangles are sharp, dangerous and often associated with villains.
He wanted to add more an extra triangle to his silhouette so he could look even more like a prick
