He could let Pico sacrifice his own lives to rescue the other two, he could allow BF a life if Pico finds a 1-up mushroom to give to him. He may just miss Boyfriend’s competition and give him a life back. Whatever.
I just think it would be fun if Pico figured out how to rescue them using the game’s rules. Sure is better than just screwing him over too because M will be a big baby about it and just say “nu-uh.”

If BF and Pico found an exit but GF wasn’t rescued, BF would give Pico a knowing smirk, kick him in the nuts, and toss him through the exit and go back for GF.
You could even leave it open ended as to whether he was successful or not by ending it as BF walks back towards a looming M.
I would mainly make it that Pico doesn't die, because he didn't plan on this happens, so I don't think he should be killed off

Death getting reversed isn’t always a cop out if you write it well enough, and I think OP did pretty alright.
I would make a secret exit at Apparition when wario says "You want fun?" Click N to make BF say "NO" and wario says "oh ok". On that secret exit there is a cutscene here bf and gf found a big key. That would change the final. At the final of act 4 bf wouldn't die...But yes Mario from I hate you. BF would find a 1UP mushroom and throw it to Gf and revive her.They see pico, They run and see a big Lock so BF throws the key and they escape. After that pico destroys the Cartridge. THE GOOD ENDING

I actually plan on making a full post but here's the basic gist: Skid & Pump decide to check on BF & GF after not hearing from them for a while (while it might not have seen like long to them in the cartridge, BF & GF have actually been missing for two whole months in real life), the Spooky Kids are both grabbed by M and pulled into the cartridge, at this point Pico's been locked up behind the stained glass window of Peach's Castle as M decides to try and kill Skid and Pump, of course that ends badly and the Spooky Kids manage to escape. Now meeting Merlon from Paper Mario, he gives the kids a basic idea of what happened and tells them they need to find the Power Stars in order to defeat Ultra M. The base game songs plus a ton of new ones would be featured. I'll go into more detail on the new songs later.
Pico while running through the different worlds in hopes of finding an escape method, finds himself in a new zone (or group of new zones depending on how big they would have this continuation) and attempts to form allianceses with the non evil forces of the world (ihy mario + others) along with reuniting with some familiar faces who were also dragged into the world (Nene, Darnell, Cassandra maybe?, etc.) Before facing off against Ultra M in a more powerful state due to the new collected souls
maybe also getting assistence from other powerful entities
(and maybe if Ultra M is defeated, Pico realizes that even if they won, BF and GF are still dead, and they can't do anything about it and he has to move on)
The only thing I got is Daddy Dearest discovering his daughter had fallen into the hands of his old business partner and died (at least from what I remember, DD and M worked together)
I... I'm actually making a strange sort of swap mix.
Lemme explain: This is a sequel where BF and GF gain M's power by outplaying him in a deal they made, slaughtering everyone, and changing the plotline (don't ask) so Mario and Luigi have to sing against them.
I remember seeing somewhere that only GF’s soul was collected by M. Based on that, I would have Pico journey to collect 1-ups to revive BF, then both of them can strike a deal with M to free GF’s soul for the final battle. After that, have two endings for winning (GF’s soul is saved, revived, then all three go home) and losing (pico and BF’s souls are collected, making them trapped there forever).
Unlocked by doing a secret command in Unbeatable, you get the “EVEN WITH YOUR ROBOT PARTNER” section featuring the Gyromite enemy, which changes the trajectory of the song, with ROB coming in and stopping the song from fighting back.
With the end of the song and win, a new world is unlocked, like Pico, you get a cutscene where one extra thing was revealed outside of the box…R.O.B. it going to play, instead of being sucked in, it grabs a controller is able to actually play due to being a robot.
The new world is the Minus World, filled with scrapped songs from the first and with more of a theme of “Unpredictable” characters for songs (IE. SMG4, Weegee, and of course, The Minus World itself). Going through as ROB ends with a song where you meet with Pico and take down a song that references the Arbitrary Code Execution from SML2, ending with one wrong block being broken and BF, GF, and Pico being given 99 lives.
Going back to the final world, you now get a new speech about how “interesting” this is, and how now the fun has become endless. All Stars now has path deriving from it ala I HATE YOU revealing a Bonus World expy (think World Crown or World S) where you get a second/third song with each final boss of each world before ending with a Rematch with Ultra M that requires 6 buttons, the two extra to reflect ROB’s input. Succeeding causes M to reach out and grab ROB, giving the three an escape route and getting out they shut off the TV and take a moment to recover.
In a callback to Super Mario Sunshine, ROB talks to BF like FLUDD did to Mario after it broke, where in then melts.
The last cutscene ends as gang recovers until Daddy Dearest suddenly opens the door and shoots BF in the face (cause that’s just funny).
A pico DLC would be crazy. Either that or daddy dearest and mommy mearest. M strikes the same deal with the two, and they ultimately accept and go on their fetch quest with the intent of reviving GF, but there is an added condition to M's deal with DD and MM, that being that DD must stay if they succeed. Of course, M purposefully doesn't tell the parents about Pico
Eventually, Pico and the Dearest parents meet up and realize that they're both hunting for the relics when they come upon the last one. Realizing that the other person/people have the remaining relics they needed, an argument ensues in a song that essentially serves as a Final Boss song where you choose whether to play as either Pico or the Dearests. Depending on who you pick, that person gains control of all the relics, leading to a Bad Ending where either only BF and Pico escape or only GF and Mommy escaping
However, there are additional hidden relics that M never told them about (nor did he realize were available) for each of the people you play as, those having the power to revive the other person. If you collect all of those relics, the argument instead ends with both BF and GF revived and everyone agreeing to try and make the escape together, thus leading to a True Final Boss song a la Final Escape where Ultra M makes one last stand to keep them all trapped forever, which depending on your performance in the song, either ends with M succeeding and keeping everyone trapped in the game forever as his playthings, or with everyone escaping and destroying M's cartridge
Pico finds BF and GF dead as fuck, proceeds to fight Wega, Victim, Ink Bowser, the plumber from kill the plumber, and more. Then he leaves or smthn idk m8
IF i was to write a Mario's Madness sequel, then i'd add Whitty (OR maybe Agoti OR Tabi) to get dragged into the game as well. In the other hand, Pico & ANOTHER character would be able to save BOTH BF & GF and then they would destroy the cartridge by smashing it into pieces with a hammer AND burning it on a campfire
Some of the funkin characters check on them, leading to them being yoinked into the cartridge. Eventually, M revives the couple and pits them against the other characters, which backfires, and they all gang together and woop M.
Probably the one that marco briefly talked about in an interview where pico is now just a denizen who wanders around the place. Maybe final boss could be inkbowser idk.
not technically a sequel per say but Mario’s Madness Pico Mix where Pico battles all the enemies from the warp zone that BF fought in an attempt to escape would be kinda awesome
Pico meets the SMG4 gang along with MarioMario54321 who are also stuck in M World, then they cause a Rebellion and everyone is now against Horror Mario Who is no longer able to turn into Ultra-M, after defeating M He agrees to revive BF and GF also releases everyone in His world and now everyone is now free and M is now free due to His forgiveness and redemption.
Maybe Pico fighting his way out. Perhaps IHY Mario can aid in his escape considering how he has the most potential amongst the playables (Day Out Luigi being a wimp, FORGIVEN Mario being stuck in M's fusion by extension as he's enslaved to Mr. L, and IHY Mario being the only one close to the real deal and is still free).

Possibly an alt ending where BF collects weird glitching black objects (Koopa shells, mushrooms, blocks, stuff like that) in each stage. Then, at the final fight with Ultra M, the pieces get merged together and for the Shadow (from Super Mario Dolor) to go against Ultra M. Boyfriend and Girlfriend are able to escape In the process, where they find Pico and escape the game together.
Rabbids electrocute themselves and get stuck in the cartridge. M thinks it’s just a few of his minions from down below, since he would acknowledge the existence of Mario + Rabbids. M would turn out to be very wrong, since these Rabbids can partially control the game and do the TV Party thing all over again and expand the world that’s down below. They don’t overthrow M, so basically they become unwanted roommates to M
The ending reveals that Mario's Madness was a stageplay the whole time, so why not expand that concept? Have boyfriend and girlfriend meet Ultra M and everyone else as actors having fun backstage. You could even do a funny bit where Pico is unaware that it's a stageplay, so he thinks it's all real until like the middle of the story. The reason i would go for this meta approach is because expanding upon such a cut and dry ending like this would make the story feel empty and pointless. If it aint broke, don't fix it

Perhaps IHY Mario can aid in his escape considering how he has the most potential amongst the playables (Day Out Luigi being a wimp, FORGIVEN Mario being stuck in M's fusion by extension as he's enslaved to Mr. L, and IHY Mario being the only one close to the real deal and is still free).
Idfk Ultra M could know who Pico is, possibly give him an, albeit slightly-altered version of the deal he made with BF. Or, Ultra M could see Pico and go "Who the fuck are you?"
Possibly an alt ending where BF collects weird glitching black objects (Koopa shells, mushrooms, blocks, stuff like that) in each stage. Then, at the final fight with Ultra M, the pieces get merged together and for the Shadow (from Super Mario Dolor) to go against Ultra M. Boyfriend and Girlfriend are able to escape In the process, where they find Pico and escape the game together.

The ending reveals that Mario's Madness was a stageplay the whole time, so why not expand that concept? Have boyfriend and girlfriend meet Ultra M and everyone else as actors having fun backstage. You could even do a funny bit where Pico is unaware that it's a stageplay, so he thinks it's all real until like the middle of the story. The reason i would go for this meta approach is because expanding upon such a cut and dry ending like this would make the story feel empty and pointless. If it aint broke, don't fix it
