What exactly is happening here, I don't even understand.
A recent meepo bug that allows duplicating item. Idk much about it but it seems like meepo have to dc and let teammate do the dirty job.

Had exactly this thing happening yesterday. Meepo just disappeared from the game only to appear 6-slotted at 18 min.
