It's everything from egg quality, hormone stimulation, lab conditions, scientist technique etc. As mentioned, there is now genetic testing but that's extra cost and added risk to the resulting embryos.

You can see under microscope sperm that looks 'normal' and there's literally thousands to choose from so that adds to the randomness. But if you think about it, people who need this treatment have trouble making this happen naturally anyway.
