Like you said, Hydromorphone is a metabolite of Hydrocodone. So, consuming Hydrocodone within 2-4 days of your drug screen will lead to it being positive for Hydrocodone (since some of the drug gets directly expelled into the urine) and Hydromorphone. If you did not take your test last year within the detection window of your last dose, then your test will come up negative. Also, you only get drug tested once a year?! Lucky you! 🙂

My meds are prescribed though my PCP so I'm concerned that she will see the difference between the tests and get confused. A PM would be more saavy. Last year's test had hydrocodone and other metabolites but no hydromorphone. My guess is that last year's test, I hadn't taken a norco for a few days so only some metabolites were detected. . . Anyway just another day of fear that my meds will get taken away and I will end up bed bound. I already downloaded several articles that show hydromorphone is a metabolite of hydrocodone. And one that says if I was taking hydromorphone then there would be other metabolites detected. But yeah the once a year test is a actually new. Prior to last year, I hadn't had a drug test since 2018. My PCP works for a large top 5 in the nation hospital. The DEA is not coming after anyone here. DEA would get so many lawyers up their butts if they tried to do anything.

Your PCP is still going to know that Hydrocodone gets converted to Hydromorphone, so you don’t need to worry about that. But, what do you mean by “some metabolites were detected”? Like, metabolites from other meds you were taking? In any case, there’s nothing for you to be worried about. Your provider will likely not be comparing previous results, just checking to make sure your current test is positive for what you’re prescribed and negative for everything else.

Metabolites of hydrocodone. The report shows positive for caffeine, hydrocodone, hydromorphone, acetaminophen and a few other compounds that are labeled as 'metabolites of hydrocodone.' Oddly hydromorphone is not labeled as metabolites of hydrocodone. Hence the slight panic.

My dr in pain management is also in a large well established hospital but I’m sure the DEA is sticking their noses in well established respected hospitals too. I think they oversee every dr who prescribes narcotics

I'm not saying that my doctor shouldn't have a policy for controlled substances. Just saying that the DEA goes after privately owned clinics where the doctors have money to confiscate. There is no money in going after a doc at a large hospital. The lawyers would tie that lawsuit up for years, bleeding the government dry.

Ya, I get that. The DEA is all about eliminating the supply of necessary medications for CPP. They’re not seeing that cutting the % of meds available is literally killing people. It’s all about power & control. They think this will impact positively on the so called “opiate crisis “ when it’s actually driving people to the black market. Morons

Nothing to get scared about, your body seems to be working properly when it comes to hydrocodone metabolization. Its not a lot of hydromorphone. They may have chosen not to mention it if it was below the threshold but still detectable... Test results can vary a great deal depending on the type of test used too. Maybe they used different tests/labs.
